To download requisition forms just click on the following link (Print a 'Physician' Request for Requisition form PDF format 186kb) and print as many forms as you need for your histology and cytology specimens. If you are a current client on our courier route your specimen will be picked up with your next scheduled run.

Patients and Physicians

To get a second opinion download a referral form (Print a 'Patient' Request for Consultation form PDF format 61kb) and fill it out. Take the completed form to your physician or laboratory. They will then forward your slides and report to me. I will revue the case and send the materials back to the original laboratory. You will receive my written consult shortly in the mail.
